Mailing Address Verification
Mailing addresses come from page 5 of the Ohio Form IT 1040 Instruction Booklet. Also, Page 1 of IT-1040 for 2004 has the addresses listed by whether or not you are enclosing a payment with your return. Good luck on finding it on their web site, which has a slightly different listing for the addresses. You have to go to the FAQs page, select "Individual Income" category, and then click on FAQ #6.
